A referral letter from your GP or referring practitioner*
Relevant investigations and imaging (if available)
Your regular exercise or sporting shoes
A pair of shorts
*Note: A referral is necessary if you are eligible for Medicare and would like to receive a rebate. Without a referral, consultations are still available but not rebatable.
